Managing exudate and preventing biofilms in community wound care

ثبت نشده
چکیده

Problems with exudate production occur if the wound becomes ‘stuck’ in the inflammatory stage and the production of exudate is overstimulated. Exudate found in chronic wounds (those that fail to heal after six weeks) has a different makeup to that in acute wounds and is often produced in higher quantities. Chronic wound exudate does not have the active growth factors found in acute wound exudate and it can actually block the healing process, destroy the extracellular matrix (ECM) and become a ‘wounding agent’ itself, causing more damage than healing (Chen et al, 1992).

Technology update: role of wound cleansing in the management of wounds

Micro-organisms produce significant barriers to the healing of chronic wounds (Kim et al, 2010). They are present in all wounds, although not all wounds become chronic. Microbes grow in both a planktonic phenotype (free-floating mobile single cells) and biofilm phenotype (a fixed polymicrobial community; James et al, 2008). A self-secreted matrix protects the biofilm from attack by the host imm...

Managing wound pain in patients with burns using soft silicone dressings Causes of pain in wound care

Causes of pain in wound care Dressing-related pain can sometimes be worse than the pain from the wound. Benbow (2010) suggests that wound and dressing-related pain is underestimated and poorly managed and may lead to extended healing times. She identifies that the most common causes of pain include removing dressings that stick to the wound bed, skin stripping of periwound skin and tissue macer...

Wound care in the community: infection, exudate and conformability

Nowhere is this truer than in the realm of wound care, where treatment choice can have a serious effect on healing and nurses often have to make ‘on-the-spot’ decisions (Hallett et al, 2000). Whereas the inpatient nurse may have access to onsite infection control teams, tissue viability specialist nurses, link nurses and various medical specialties such as vascular, plastic surgery, surgical, e...

The effect of a hydroconductive dressing on the suppression of wound biofilm.

UNLABELLED Although excessive exudate has been associated with poor wound healing outcomes, exudate is still not well understood in the pathophysiology of chronic wounds. Wound exudate is believed to be the result of wounds that are trapped in a persistent, hyper inflammatory state. Biofilm, bacteria of multiple species living in community, has multiple well-defined molecular pathways that prod...

What Kind of Wounds Are Seen in the Community?

Wounds commonly seen in the community include leg ulcers, pressure ulcers and diabetic foot ulcers, particularly when these become chronic and non-healing, which can involve nurses having to manage infection and varying exudate volumes. Managing these non-healing wounds in the community can be expensive, not only in terms of resources such as nursing time and dressing costs, but, also on patien...
